Spontaneous Consecutive Exotropia.

Abstract
Consecutive exotropia can occur spontaneously in young children with accommodative esotropia.We report the findings of such patients. We reviewed the medical records of children with whom consecutive exotropia developed spontaneously after correction of hyperopia because of accommodative esotropia. Four patients [2 girls and 2 boys] were studied.The mean age at onset of the esotropia was 2.1 years. The mean degree of hyperopia was 4.77 diopters [D] [range 2.75~6.75D, spherical equivalent], and after the childen had started to wear glasses all of them showed orthophoria with correction. At the mean age of 6.3 years the exotropia became evident. The mean amount of exodeviation was 21.0 prism diopters [PD] with correction and 9.7PD without correction. Refractive error varied from 0.63D to 6.00D with a mean of 3.56D. Three children had present or previous amblyopia. A high degree of hyperopia seemed to predispose to the spontaneous exotropia. In conclusion,possible development of spontaneous consecutive exotropia should be explained to the parents of accommodative esotropia children.
