OPA1 유전자의 c.1334G>A 돌연변이에 의해 발생한 우성시신경위축 환자

목적

우성시신경위축은 가장 흔히 볼 수 있는 유전시신경병증 중 하나로, 10대부터 서서히 진행하는 양안 중심시력저하를 보인다. OPA1 유전자 변이로 인한 미토콘드리아 기능장애가 증상을 발현시키며, 저자들은 OPA1 유전자의 c.1334G>A 돌연변이로 인한 우성 시신경위축 증례를 경험하여 이를 국내에서 처음으로 보고하고자 한다.

증례요약

특이 병력과 전신 증상이 없는 12세 여아가 서서히 진행하는 양안의 시력저하를 주소로 내원하였다. 첫 내원 당시 세극등현미경검사, 안압, 안구운동, 동공반사는 정상이었으나, 양안 최대교정시력이 20/100이었고, 이시하라 색각검사표에서 양안 8개로 저하되어 있었다. 안저검사에서 양안 시신경유두의 이측 창백이 관찰되었고, 골드만시야검사에서 이에 상응하는 주시점맹점암점을 보였다. 빛간섭단층촬영에서 양안 유두주위 망막신경섬유층과 황반부 신경절세포층의 두께가 현저하게 얇아져 있었으며, OPA1 유전자의 염기서열 1,334번째 염기가 G에서 A로 바뀐 c.1334G>A 돌연변이가 확인되었다.

결론

OPA1 유전자의 c.1334G>A 돌연변이로 인해 발생한 우성시신경위축의 특징적인 임상증상을 본 증례를 통해 보고하고자 한다.

ABSTRACT

Purpose

Dominant optic atrophy is one of the most common hereditary optic neuropathies, causing progressive bilateral vision loss that begins early in life. Optic atrophy 1 (OPA1) gene mutation brings about mitochondrial dysfunction, which results in clinical manifestations of dominant optic atrophy. Here, we report a case of dominant optic atrophy caused by the c.1334G>A mutation of the OPA1 gene, the first known case in Korea to our knowledge.

Case summary

A 12-year-old female patient with no specific medical history or systemic symptoms visited our clinic complaining of a progressive decrease in vision in either eye. Slit-lamp microscopy, intraocular pressure, ocular motility, and pupil reflex were normal. However, her best-corrected visual acuity in both eyes was 20/100, and her color vision was reduced to 8/12 in Ishihara’s test. Fundus examination showed temporal pallor of the optic nerve head in both eyes, and a corresponding cecocentral scotoma was observed on Goldmann visual field examination. Optical coherence tomography revealed significant thinning of the peripapillary retinal fiber layer and macular ganglion cell layer in both eyes. Genetic examination confirmed the c.1334G>A mutation of the OPA1 gene.

Conclusions

We report a case of dominant optic nerve atrophy caused by c.1334G>A mutation of the OPA1 gene and its clinical manifestations.

우성시신경위축(dominant optic atrophy)은 레버유전시신경병증과 더불어 가장 흔하게 진단되는 유전시신경병증 중 하나이며, 유병률은 1:12,000에서 1:50,000으로 다양하게 보고되어 있다[1-4]. 우성시신경위축 환자의 전형적인 임상증상은 20세 이전의 젊은 연령에서 서서히 진행하는 양안의 무통성 중심시력저하 및 색각저하이며, 안저검사에서 양안의 시신경유두의 이측 창백(temporal pallor)과 유두주위 망막신경섬유층(peripapillary retinal nerve fiber layer)의 두께가 감소한 소견이 관찰된다[3-6].
미토콘드리아 DNA의 돌연변이로 인해 발병하는 레버유전시신경병증과 달리, 우성시신경위축은 핵 DNA의 돌연변이에 의해 발생하며, 우성시신경위축 환자의 대다수는 3번 염색체의 장완에 존재하는 OPA1 유전자의 돌연변이에 의해 발생한다고 보고되었다[3,4]. OPA1 유전자에 의해 생성되는 OPA1 단백질은 미토콘드리아 내막의 융합에 필수적인 역할을 하며, 미토콘드리아 내의 oxidative phosphorylation complex와 상호작용하여 adenosine triphosphate를 생성하고 미토콘드리아 내막 유지에 중요한 역할을 한다[7-10]. OPA1 유전자의 돌연변이가 발생할 경우, OPA1 단백질의 생성이약 50%까지 감소하게 되며, 그 결과 미토콘드리아 기능 저하(mitochondrial dysfunction)가 유발된다[3,11,12]. 미토콘드리아의 기능저하가 발생하면 다른 기관의 세포들보다 에너지 요구량이 많고, 해부학적․생리적 이유로 미토콘드리아 기능 저하에 가장 취약한 망막신경절세포가 선택적으로 손상되어 우성시신경위축 환자에서 양안의 중심시력저하 및 시신경유두의 이측 창백 소견 등의 특징적인 임상 양상이 관찰된다[4,8,12,13].
저자들은 OPA1 유전자의 c.1334G>A 돌연변이에 의해 발생한 우성시신경위축 증례를 진단하여 이를 국내 최초로 보고하고자 한다.

증례보고

특이 병력과 전신 증상이 없는 12세 여아가 서서히 진행하는 양안 중심시력저하를 주소로 내원하였다. 환아는 2016년 5월, 학교에서 받은 신체검사를 통해 우연히 양안 시력저하를 발견하고 안경점에서 안경을 맞추어 착용하기 시작하였으나 교정시력이 잘 나오지 않아 병원 방문을 권유받고 본원에 내원하였다. 2017년 3월, 외래 첫 방문 당시 세극등현미경검사, 안압, 안구운동, 동공반사는 정상이었으나 양안 최대교정시력은 20/100이었으며, 이시하라 색각검사표에서 양안 각각 8개만 확인 가능하였다. 안저검사에서 양안 시신 경유두의 이측 창백 소견이 확인되었으며(Fig. 1), 골드만시야검사에서 이에 상응하는 주시점맹점암점을 보였다(Fig. 2). 또한, 빛간섭단층촬영에서 양안의 유두주위 망막신경섬유층과 황반부의 신경절세포층의 두께가 현저하게 얇아진 것을 확인하였다(Fig. 3). 혈액학적 검사상 complete blood cell count, liver function test, renal function test, angiotensin converting enzyme, antinuclear antibodies, aquaporin 4, fluorescent treponemal antibody absorption test 결과는 모두 정상이었으며 뇌자기공명영상, 신체 검진과 신경학적 검사 또한 정상이었다. 독성물질에 노출된 적은 없었고, 질병으로 인한 약물 복용력 및 두부 외상의 병력 또한 없었다.
우성시신경위축의 특징적인 임상 소견을 보였기에, 환아의 말초혈액의 백혈구를 통해 추출된 DNA로 next-generation sequencing 방법에 의한 유전성 안질환유전자 패널 검사를 시행하였으며, 그 결과 OPA1 유전자의 염기서열 1,334번째 염기가 G에서 A로 바뀐 c.1334G>A 돌연변이를 확인하였고, Sanger sequencing에서도 heterozygote임을 확인하였다. 유전성 안질환유전자 패널검사에 포함된 OPA3, WFS1, MFN2, ACO2, AFG3LS, C12ORF65, CISD2, NR2F1, SPG7, TIMM8A, TMEM126A 유전자의 돌연변이는 관찰되지 않았다. 레버유전시신경병증의 원인인 미토콘드리아 DNA 의 대표적인 돌연변이 3종은 관찰되지 않았으며, 추가로 근육퇴행위축 및 근병증에 관한 유전자 돌연변이를 확인하기 위하여 GNE 유전자와 DYSF 유전자 검사를 시행하였으나 돌연변이는 확인되지 않았다. 환아의 부모는 유전자검사에 동의하지 않아 OPA1 유전자의 변이 여부를 확인할 수 없었으나, 원인불명의 시력저하에 대한 가족력은 없었다. 상기 환아를 3년간 추적 관찰하는 동안, 양안 최대교정시력은 20/100으로 유지되었고, 색각 및 망막신경섬유층의 두께는 변화가 없었으며, 청각장애나 신경학적 소견 등의 전신적인 이상은 아직 나타나지 않았다.

고 찰

우성시신경위축의 발생 원인으로 알려진 OPA1 유전자 돌연변이는 현재까지 약 500개 이상의 돌연변이 종류가 보고되어 있으나, 각각의 돌연변이가 유발하는 임상적 유의성은 아직 명확하게 밝혀지지 않은 상태이다[1-3]. OPA1 유전자에 돌연변이가 발생하면 haploinsufficiency 기전에 의해 OPA1 단백질 생성이 정상보다 약 50%까지 감소하게 된다[3,11-13]. 이는 미토콘드리아 기능저하를 유발하게 되어 우성시신경위축의 특징적인 임상증상인 양안의 중심시력저하와 색각저하 및 시신경유두의 이측 창백이 발현된다. 조직병리학적으로는 미토콘드리아 기능저하에 선택적으로 취약한 시신경유두황반다발(papillomacular bundle)의 망막신경절세포(retinal ganglion cell)의 세포 손상이 관찰된다[4,8,12,13].
우성시신경위축 환자들의 임상증상은 같은 종류의 돌연변이를 가지고 있는 동일 가족 안에서도 incomplete penetrance 기전으로 인해 각기 다른 양상을 보일 수 있다[4]. 우성시신경위축 환자들의 시력은 광각유의 심한 시력소실에서 20/20의 정상시력까지 다양하게 분포한다. 대부분 환자들의 시력은 20/200 이상으로 나타났는데, 우성시신경위축 환자들의 약 3분의 1에서는 최종시력이 20/60 이상으로, 약 2분의 1에서는 20/200과 20/60 간의 시력을 가지며, 나머지 에서는 20/200 미만의 최종시력이 나타났다고 보고되었다[4,5]. 레버유전시신경병증과 달리 우성시신경위축 환자에서 시력의 자연 호전은 문헌에 보고된 적이 없다[4,12].
우성시신경위축 환자의 약 20%에서는 안과적 증상 외에 청력 저하, 근병증, 말초신경병증, 만성진행외안근마비(chronic progressive external ophthalmoplegia) 등의 전신증상이 동반되는 ‘우성시신경위축 plus syndrome’이 나타난다[1]. 이들은 전신증상이 없는 우성시신경위축 환자에 비해 시력저하가 훨씬 더 심하며, 시력저하가 발생하고 난 뒤, 수십 년이 지난 후 주로 30-40대에서 전신증상들이 나타난다[5,12,13].
본 증례에서 관찰된 c.1344G>A 돌연변이는 OPA1 유전자의 445번째 아미노산인 arginine을 histidine으로 변화시키는 과오 돌연변이(missense mutation)이다[6]. OPA1 유전자는 dynamin 관련 단백질을 암호화하는 핵 유전자이며 dynamin은 미토콘드리아 융합에 관여하는 보존된 종류의 단백질로 알려져 있다[1-4]. 문헌에 보고된 OPA1 유전자의 대부분의 과오 돌연변이는 GTPase domain에 존재하며, GTPase domain은 Dyn1에서 분자 내 또는 분자 간 상호작용에 관여하는 것으로 알려져 있다[1-4]. GTPase domain에 발생하는 돌연변이는 GTPase의 활성과 다른 단백질과의 상호작용도 손상시킬 수 있으며[12,14], Barboni et al [6]은 우성시신경위축을 유발하는 돌연변이의 종류 중에서 과오돌연변이가 다른 종류의 돌연변이(premature translation termination, splice variant, frame shift, nonsense, deletion or duplication errors)보다 상대적으로 심한 임상증상을 나타낸다고 보고하였다. Payne et al [14]은 양안의 시신경위축 및 청력 손상, 안검하수 및 외안근마비를 보이는 가계의 30명에서 유전자검사를 시행하였으며, 이 중 18명에서 OPA1 유전자의 c.1344G>A 돌연변이를 발견하였고 상기 돌연변이는 대조군 200명에서는 관찰되지 않았다고 보고하였다.
우성시신경위축의 plus syndrome은 다른 유전질환들과 임상적인 유사점을 보일 수 있어 환자 진단 시 주의가 필요하다. 예를 들면, WFS1 유전자의 돌연변이로 인해 발생하는 Wolfram syndrome 환자는 양안의 시신경위축과 함께 난청, 내당능장애, 비뇨기과적 이상 등이 동반되며, ANT1 유전자의 돌연변이가 있는 환자에서는 외안근 마비를 나타 낸다[4,15].
본 증례의 경우, 12세 환아에서 서서히 발생한 양안의 중심시력저하로 시행한 안저검사에서 양안 시신경유두의 이측 창백이 관찰되었고, 골드만시야검사에서 이에 상응하는 중심부근암점을 보였으며 빛간섭단층촬영에서 양안의 유두 주변 망막신경섬유층의 두께가 감소하여 우성시신경위축의 전형적인 임상증상을 관찰할 수 있었다. 또한, 유전자 검사에서 OPA1 유전자의 c.1334G>A 돌연변이를 확인함으로써 우성시신경위축을 분자생물학적으로 확진할 수 있었다. 약 3년간 상기 환아를 추적 관찰하는 동안 감각신경성 난청을 의심할 만한 증상은 나타나지 않았다. 하지만, 여러 문헌에서 보고한 바와 같이 OPA1 유전자에 c.1334G>A 돌연변이가 발생한 우성시신경위축 환자에서는 추후 30-40대에 청력손상, 근병증 등의 전신적 증상이 동반될 수 있으므로 이를 환자 및 환자 보호자에게 설명하고 정기적인 추적 관찰이 필요할 것으로 생각된다. 이에 저자들은 상기 우성시신경위축 증례를 전형적인 임상 소견과 함께 국내에서 최초로 문헌에 보고하고자 한다.
