Abstract

목적

생체 외에서 배양된 사람 각막내피세포에서 cyclosporine A (CsA)로 인한 노화의 영향에 대해 알아보고자 하였다.

대상과 방법

사람 각막내피세포를 배양하고 0-100 µM CsA와 incubation을 시행하였다. 노화 연관 베타 갈락토시다제(senescence-associated β-Galactosidase, SA-β-gal) 염색을 시행하였다. WST-8 분석 키트를 사용하여 미토콘드리아 탈수소효소의 활성을 평가하고, JC-1 dye를 이용하여 미토콘드리아막 전위(ΔΨm)를 측정하였다. 세포 내와 미토콘드라아에서 활성 산소 형성을 측정하였다. 세포 내와 미토콘드리아의 칼슘 수치를 각각 Fluo-4와 Rhod-2를 통해 측정하였다. 단백질 발현은 웨스턴 블롯(western blot)에 의해 평가되었다.

결과

농도 의존적으로 CsA는 SA-β-gal 양성 세포의 비율을 증가시켰고(p=0.003) 미토콘드리아 탈수소효소의 활성과 ΔΨm을 감소시켰다(p=0.029, p=0.004). 세포 내 및 미토콘드리아의 활성 산소는 CsA와 배양하는 동안 증가하였다(p=0.005). 100 μM의 CsA에서 미토콘드리아 칼슘 수치가 높아진 반면(p=0.001), 세포 내 칼슘 수치는 낮아졌다(p=0.029). CsA는 GSK3β와 ERK1/2를 활성화시키고 ZO-1 발현을 감소시켰다.

결론

사람 각막내피세포에서 CsA는 산화 스트레스의 유도와 미토콘드리아, GSK3β 및 ERK1/2 경로를 통해 노화를 유도한다. 따라서 고농도의 CsA 사용시 주의하여 사용하여야 한다.

Trans Abstract

Purpose

To investigate the in vitro effect of cyclosporine A (CsA)-induced senescence on human corneal endothelial cells (HCECs).

Methods

HCECs were cultured and incubated with 0-100 µM CsA. Senescence-associated β–galactosidase (SA-β-gal) staining was performed. Mitochondrial dehydrogenase activity was assessed using a WST-8 assay kit and mitochondrial membrane potential (∆Ψm) was measured using JC-1 dye. Intracellular and mitochondrial formation of reactive oxygen species (ROS) was measured with 2’,7’-dichlorodihydrofluorescein diacetate and MitoSOX probes. Intracellular and mitochondrial calcium levels were measured using Fluo-4 and Rhod-2, respectively. Protein expression was evaluated by Western blotting.

Results

CsA increased the percentage of SA-β-gal-positive cells (p = 0.003) and decreased mitochondrial dehydrogenase activity and ∆Ψm in a dose-dependent manner (p = 0.029, p = 0.004). Intracellular and mitochondrial ROS levels increased during incubation with CsA (p = 0.005). CsA at 100 µM increased mitochondrial calcium levels (p = 0.001), whereas intracellular calcium levels decreased at 100 µM CsA (p = 0.029). CsA activated GSK3β and ERK1/2 and reduced ZO-1 expression.

Conclusions

CsA induces senescence in HCECs through oxidative stress and via mitochondria-, GSK3β-, and ERK1/2-dependent pathways. Thus, concentrations of CsA should be monitored.

각막내피세포는 각막기질을 탈수시키고 투명성을 유지한다[1,2]. 각막내피세포는 대사적으로 활성적이며, 각막내피세포의 주요 기능 중 하나는 펌프를 통해 각막기질에서 이온과 유체(fluid)를 각막기질 밖으로 배출시키는 것이다[1]. 나트륨-칼륨 아데노신트리포스파타제(Na+-K+ ATPase)는 각막내피세포의 기저측 막에 위치하며, 각막내피의 펌프 기능에 기여한다[3]. 각막내피세포는 Na+-K+ ATPase 펌프로 인해 다량의 ATP를 소비한다[3]. ATP는 일반적으로 미토콘드리아의 내부 막(inner membrane)에서 생성된다[4]. 건강한 미토콘드리아의 기능은 각막내피세포의 펌프 기능에 중요하다[5]. 칼슘 과부하, 산화 스트레스 및 최근에 인지된 소포체(endoplasmic reticulum, ER) 스트레스는 노화, 푹스각막내피이상증(Fuchs’ endothelial dystrophy), 수포각막병증(bullous keratopathy) 등 각막내피 질환과 초음파수정체유화술(ultrasound phacoemulsification)로 인한 각막내피 손상에서 보이는 병리학적 변화를 특징짓는다[2,6,7].

노화 과정은 각막의 투명성과 건강에 영향을 줄 수 있다[8]. 노화는 각막내피세포 소실의 중요한 원인 중 하나이다[8]. 노화는 각막부종으로 인해 각막 이식이 필요한 중요한 원인이다[9]. 따라서 노화는 각막내피 질환의 치료를 위한 주요 표적일 수 있다. 노화된 세포는 크기가 더 크고, 따라서 노화되지 않은 세포에 비해 더 큰 다면성(polymegathism)을 보인다[6,10,11]. 또한 노화에서는 성장 정지와 전체 세포 수의 감소가 특징적이다[12]. 배양된 정상 섬유아세포에서 세포 자연사는 노화가 될수록 더 빈번하다[12]. 산화 스트레스는 노화를 유발하는 것으로 잘 알려져 있다[13]. 미토콘드리아/산화 스트레스 경로는 노화와 관련된다[14].

Cyclosporine A (CsA)는 자가 면역 질환의 치료 및 동종 이식편의 면역학적 거부 예방에 일반적으로 사용되는 면역 억제제이다[15]. CsA는 이식편의 생존 및 각막 이식 수여자의 삶의 질을 향상시키는 것으로 보고되었다[15]. 칼슘/칼시뉴린 신호 전달 경로(calcium-calcineurin signaling pathway)는 효과적인 면역 반응에 필요하다[16]. CsA는 세포질의 사이클로필린(cyclophilin)에 결합하여 칼시뉴린을 억제한다; 칼시뉴린의 억제는 면역 세포 증식을 억제한다[17]. CsA는 각막 이식뿐만 아니라 안구건조증 및 알레르기결막염을 포함한 안구 표면 염증에 사용되고 있다[18,19]. 안구건조증 환자에서 0.05% CsA 점안은 임상적으로 각막내피에 변화를 일으키지 않는 것으로 보고된 바 있으나, 이 연구에서는 사람 각막내피세포의 세포 밀도와 형태학적 영향만 조사했다[20]. 또한 오직 낮은 농도의 CsA만 보고되었다. 지금까지 사람 각막내피세포의 노화에서 CsA의 영향에 대한 연구는 이루어지지 않았으므로 사람 각막내피세포에서 CsA의 영향에 대한 연구가 필요하다. 이번 연구에서는 배양된 사람 각막내피세포의 노화에 대한 CsA의 효과에 대해 알아보고자 하였다.

대상과 방법

사람 각막내피세포의 분리 및 배양

본 연구는 한림대학교 강남성심병원 임상시험심사위원회(Institutional Review Board, IRB)/윤리위원회에서 검토 및 승인하였다(승인 번호: 2019-12-008). 이 연구는 헬싱키선언에 따라 수행되었다. 세포는 이전에 기술된 방법으로 배양되었다 [15, 21]. 3-6차례 계대 배양을 거친 세포가 사용되었다[2]. 세 명의 공여자로부터 얻은 각막이 사용되었다. 배양된 사람 각막내피세포를 0, 1, 10, 100 μM의 CsA와 함께 2시간 동안 배양하였다. 각막은 Lions Eye Bank (Portland, OR, USA)에서 구입하였으며, 모든 조직 샘플에 대해 사전 동의를 구하였다[21].

노화 베타 갈락토시다제(β-Galactosidase) 염색

Senescence-associated β-galactosidase (SA-β-gal) 염색 키트(BioVision, San Francisco, CA, USA)를 사용하여 세포의 노화를 평가하였다[22]. 요약하면, 세포로부터 성장 배지를 제거한 후 세포를 phosphate-buffered saline (PBS)로 세척하였다. 실온에서 세포를 15 분 동안 고정 용액에 고정시켰다[22]. 세포를 PBS로 세척하고, SA-β-gal 염색 용액을 사용하여 37°C에서 밤새 배양하였다.

WST-8 키트를 사용한 미토콘드리아 탈수소효소(mitochondrial dehydrogenase)의 활성 분석

세포를 96-well plate에서 2시간 동안 0-100 μM CsA로 처리하였다. 미토콘드리아 탈수소효소의 활성은 WST-8 키트(Dojindo, Osaka, Japan)로 측정되었으며, microplate reader (Spectramax Plus 384, Molecular Devices, San Jose, CA USA)를 사용하여 450 nm에서 흡광도를 측정하였다[2]. 미토콘드리아 탈수소효소의 활성은 대조군(100%)의 백분율(평균 ± 표준 편차)로 표현되었다[2].

미토콘드리아 막 전위(ΔΨm)의 평가

미토콘드리아 막 전위(ΔΨm)는 마이크로플레이트 분석(microplate assay)에서 JC-1 염료(BioVision, San Francisco, CA, USA)를 사용하여 측정되었다[23]. Phenol red-free 배지에서 Black 96-well plates에 seeding된 세포를 2시간 동안 37°C에서 0-100 μM의 CsA로 처리하였다. JC-1을 사용하여 사람 각막내피세포에서 ΔΨm의 변화를 입증하였다. 각 농도의 CsA에 대해 4개의 well을 사용하였다. Corresponding blank를 제외한 후 형광의 상대 강도를 계산하였다[24]. JC-1의 적색/녹색 형광 강도의 비를 계산하였다[23].

세포 내와 미토콘드리아에서의 reactive oxygen species (ROS) 생성 평가

세포 내와 미토콘드리아에서의 ROS 생성은 참고 문헌에 따라 microplate assay에서 dichloro-dihydro-fluorescein diacetate (DCFH-DA; Invitrogen, Carlsbad, CA, USA)와 MitoSOX (Invitrogen)를 사용하여 평가되었다[2]. CsA의 각 농도에 대해 6개의 well을 사용하였다. DCF 형광의 상대 강도를 계산하였다[24]. 5 μM의 최종 농도에서 MitoSOX가 well에 첨가되었다.

미토콘드리아 및 세포질의 칼슘 수치 평가

미토콘드리아 및 세포질의 칼슘 수치는 microplate assay에서 각각 Rhod-2 AM (Invitrogen)와 Fluo-4 AM (Invitrogen)을 사용하여 측정되었다[25]. Rhod-2 AM (1 μM) 또는 Fluo-4 (1 μM)가 추가되었으며, 그 후 어두운 곳에서 20분 동안 37°C로 배양하였다. 형광 강도를 측정하기 위해 분광계(spectrofluorometer)를 사용하였다. 각 농도의 CsA에 4개의 well이 사용되었다. 형광의 상대 강도를 계산하였다[23,24].

세포 주기 분석

NucleoCounter® (NC-3000™, ChemoMetec, France)에 의한 세포주기 분석은 다음과 같이 수행되었다[26]. 즉, 세포를 1 × 106 cells 밀도로 플레이팅하고 2시간 동안 배양하였다. 24시간 후, 부유 세포 및 접착성 세포가 얻어졌다. 세포를 차가운 70% 에탄올로 고정시키고, 0.1% Triton X-100, RNase A (30 μg/mL), 0.1% ethylenediaminetetraacetic acid 및 프로피듐 아이오다이드(propidium iodide) (50 μg/mL) (pH 7.4)을 함유한 염색 용액과 함께 배양하였다[26]. NucleoCounter® (NC-3000™)를 사용하여 세포 DNA 함량을 분석했다[26]. 세포 주기 분포 분석에는 최소 10,000개의 세포가 사용되었다[26].

웨스턴 블롯(Western blot)에 의한 단백질 발현 분석

웨스턴 블롯은 표준 기법을 사용하여 수행되었다[21]. 간략히 기술하자면, 세포 단백질은 phosphatase inhibitor cocktail (PhosSTOP; Roche, Basel, Switzerland)와 protease inhibitor cocktail (Sigma-Aldrich, St. Louis, MO, USA)을 포함한 radioimmunoprecipitation assay buffer (Biosesang, Seoul, Korea)를 사용하여 추출되었다[21]. 15분 동안 16,000 × g에서 원심 분리한 후 상층액을 glycogen synthase kinase (GSK) 3β, phosphorylated glycogen synthase kinase (pGSK) 3β, extracellular signal-regulated kinase-1/2 (ERK1/2) 및 phosphorylated extracellular signal-regulated kinase-1/2 (pERK1/2)의 측정에 사용하였다. 세포 단백질에 sodium dodecyl sulfate polyacrylamide gel electrophoresis를 실시하고, polyvinylidene difluoride 막(Immobilon-P; Millipore Corp., Bedford, MA, USA)으로 옮기고 5% 탈지유에서 30분 동안 배양하였다. 그 다음 anti-human mouse ERK1/2 antibody (Abcam, Cambridge, MA, USA), anti-human pERK1/2 rabbit antibody (Abcam), anti-human GSK3β rabbit antibody (Abcam) 또는 anti-human pGSK3β rabbit antibody (Abcam)와 같은 일차 항체와 함께 배양하였다. 5-bromo-4-chloro-3-indolylphosphate/nitro blue tetrazolium 발색 기질(Promega, Madison, WI, USA)은 이차적 알칼리-인산분해효소(alkaline-phosphatase) 접합 항체와 함께 3시간 동안 배양된 후 면역 반응성 단백질 밴드의 검출에 사용되었다. 데이터를 정량화하기 위해 이미지 분석 시스템(Chemidoc XRS, Bio-Rad, Hercules, CA, USA)을 사용했다[21].

Zoula occludens 1 (ZO-1)의 면역 형광 염색

사람 각막내피세포를 24-well 배양 플레이트에 seeding하고 PBS에서 3.7% 포름알데히드 용액에 15분 동안 고정시켰다[2]. 실온에서 5% 염소 혈청과 함께 30분 동안 배양한 세포를 2시간 동안 anti-human ZO-1 rabbit antibody (Santa Cruz Biotechnology, Santa Cruz, CA, USA)와 함께 배양하였다. 이어서, 플루오레세인 이소티오시안산염(fluorescein isothiocyanate)이 접합된 anti-rabbit goat IgG antibody (1:100)와 함께 2시간 동안 어둠 속에서 실온 배양하였다. 핵 염색은 Hoechst 33342 dye (1:1000 희석; Invitrogen)로 시행되었다[21].

통계 분석

그룹 간 차이는 Kruskal-Wallis test와 Mann-Whitney U test로 평가하고, p<0.05의 차이는 통계적으로 유의한 것으로 간주되었다. 모든 검사는 3세트씩 시행되었으며 모든 데이터는 평균 ± 표준 편차로 표시하였다.

결 과

사람 각막내피세포의 식별 및 배양된 사람 각막내피세포의 노화

P0에서 배양된 1차 각막내피세포는 생체 내에서 보이는 것과 유사한 육각형 형태를 보였다. 세포는 모자이크 패턴을 보여주었다(Fig. 1A) [21]. SA-β-gal 염색에 의해 노화를 측정하였다. SA-β-gal-양성 세포의 백분율을 CsA의 농도에 따라 증가시켰다(p=0.003, Kruskal-Wallis test) (Table 1, Fig. 1B, C). SA-β-gal-양성 세포의 백분율은 대조군과 비교하여 10, 100 μM에서 높았다(p=0.029, Man-Whitney U test). 세포주기 분석은 CsA가 S기의 세포 백분율을 감소시켰음을 보여주었다(p=0.007) (Table 1, Fig. 1D, E). CsA는 대조군에 비해 1, 10 및 100 μM에서 S기의 세포 백분율을 감소시켰다(p=0.029, Mann-Whitney U test). S기 세포의 백분율은 100 μM에서 1 μM에 비해 더 낮았다(p=0.029).

The effect of cyclosporine A on cultured human corneal endothelial cells

미토콘드리아 탈수소효소 활성 및 ΔΨm에 대한 CsA의 효과

WST-8 분석에 의해 평가된 미토콘드리아 탈수소효소의 활성분석에서, CsA는 대조군과 비교하여 100 μM에서 미토콘드리아 탈수소효소의 활성을 감소시켰다(p=0.029, Mann-Whitney U test) (Table 1, Fig. 2A). JC-1을 사용하여 ΔΨm을 측정하였다. CsA는 농도가 증가함에 따라 ΔΨm을 감소시켰다(p=0.004, Kruskal Wallis test) (Table 1, Fig. 2B). ΔΨm은 대조군과 비교하여 10 μM 및 100 μM에서 더 낮았다(p=0.029, Man-Whitney U test). ΔΨm은 대조군, 1 μM 및 10 μM 농도의 CsA와 비교하여 100 μM에서 더 낮았다(p=0.029).

세포 내와 미토콘드리아에서 ROS에 대한 CsA의 효과

세포 내 ROS 형성은 DCF 형광에 의해 측정되었다[24]. 세포 내 ROS는 CsA의 농도에 따라 증가하였다(p=0.005, Kruskal Wallis test) (Table 1, Fig. 2C). CsA는 10 μM에서 대조군과 1 μM에 비하여, 100 μM에서는 대조군, 1 μM, 10 μM에 비하여 세포 내 ROS 수치를 상승시켰다(p=0.029). 미토콘드리아 ROS 형성은 MitoSOX red 형광에 의해 측정되었다. CsA는 대조군, 1 μM, 10 μM과 비교하여 100 μM에서 미토콘드리아의 ROS 수치를 상승시켰다(p=0.029) (Table 1, Fig. 2D).

세포 내 및 미토콘드리아의 칼슘에 대한 CsA의 영향

세포 내 칼슘은 Fluo-4에 의해 측정되었다[25]. CsA는 100 μM에서 대조군보다 칼슘 수치를 감소시켰다(p=0.029) (Table 1, Fig. 3A, B). 미토콘드리아의 칼슘 수치는 Rhod-2에 의해 측정되었다[25]. Rhod-2 형광 강도는 CsA의 농도가 높아짐에 따라 증가하였다(p=0.008, Kruskal-Wallis test) (Table 1, Fig. 3C, D). CsA는 대조군과 비교하여 1 μM에서 미토콘드리아 칼슘 수치를 감소시켰고 100 μM에서는 상승시켰다(p=0.022, p=0.001).

웨스턴 블롯에 의한 단백질 발현 분석

웨스턴 블롯을 통해 CsA로 치료받은 사람 각막내피세포에서 GSK3β 및 ERK1/2의 발현을 확인하였다(Fig. 4A-C). CsA는 GSK3β와 ERK1/2의 인산화를 증가시켰다(p=0.007, p=0.018, Kruskal-Wallis test).

면역 형광 염색에 따른 ZO-1 발현

ZO-1 (녹색)에 대해 면역 형광 염색을 시행하였다. CsA는 농도 의존 방식으로 ZO-1 발현을 감소시켰다(Fig. 4D).

고 찰

일정한 수의 세포분열 후, 정상 세포 증식은 비가역적으로 정지되고, 이 현상을 노화라고 한다[13]. 노화 세포는 대사적으로 계속 기능을 하지만, 제한된 세포 분열, 감소된 성장 속도, 평평하고 확대된 세포 형태 및 세포주기 조절 단백질의 변화를 보인다[27]. 이 연구에서, CsA는 배양된 사람 각막내피세포에서 노화를 유도하였다. CsA로 처리한 후 노화 세포의 수는 농도 의존적으로 증가하였다. 세포주기 분석은 S기의 세포 수가 적어 CsA가 세포주기 정지를 유도함을 보여주었다. S기 정지에서는 G0/G1 단계에서 축적되는 노화 세포에서 말단 세포주기 정지를 보인다[28].

CsA은 안구 염증 질환의 국소적 치료 외에도 건선, 류마티스성 관절염과 같은 자가면역 질환의 치료나 간, 콩팥, 심장 등의 장기 이식 후 면역억제를 위해 사용되고 있다[29]. 기존의 몇몇 연구에서는 CsA로 인한 만성 신부전과 같은 신독성과 세포 노화의 유도가 보고되었다[30-32]. Justo et al [33]은 배양된 콩팥세관 상피세포에서 10 μg/mL, 8.3 mM CsA에 의해 여러 단계의 apoptotic pathway가 유도됨을 확인하였으며, 주요한 apoptotic pathway로 mitochondrial injury를 보고하였다. Mandavilli et al [34]은 미토콘드리아 DNA의 손상이 회복되지 않으면, 변화된 효소가 electron transport chain에 도입됨으로써 자유 라디칼로 인한 손상을 가속화시키고 궁극적으로 더 많은 미토콘드리아 DNA 돌연변이 및 oxidant production을 초래한다고 하였다. 결국 미토콘드리아 ROS의 생산과 DNA 돌연변이의 악순환은 세포의 노화로 이어질 수 있다[34].

지금까지 전신 질환의 치료에서 CsA의 복용으로 인한 각막내피세포의 노화에 대한 연구는 아직 보고된 바 없다. 본 연구는 배양된 사람 각막내피세포에서 CsA에 의한 노화의 효과에 대해 분석하였다. 이 연구에서 우리는 CsA와 사람 각막내피세포 배양 후 미토콘드리아의 변화에 초점을 맞추었는데 미토콘드리아는 세포 내 ROS의 주요 공급원이며 노화 과정에서 필수적인 역할을 한다[35]. 노화 중에 미토콘드리아 기능이 중단되며, 미토콘드리아 기능은 ΔΨm의 감소 및 과산화물 생성의 증가에 의해 평가될 수 있다[36].

ΔΨm은 ATP 생성이라는 미토콘드리아의 생리 기능 유지에 중요하다[37]. 투과성 전이 세공 개방(permeability transition pore opening)은 미토콘드리아 기능에 비가역적으로 유해한 영향인 것으로 보고되었다[36,38]. 투과성 전이 세공 개방은 후속 ΔΨm의 손실, 매트릭스 이온의 방출 및 세포질로의 미토콘드리아 대사의 손실을 포함한다[38]. ΔΨm의 현저한 손실은 에너지를 고갈시키고 결과적으로 세포 사멸을 초래한다[36]. 고용량의 산화 스트레스는 ΔΨm의 후속 손실을 유발할 수 있다[39]. 본 연구에서, 배양된 각막내피세포에서 고농도의 CsA는 농도 의존적 방식으로 ΔΨm을 감소시켰다.

본 연구에서 CsA가 세포 내 ROS 수치를 증가시키는 것으로 밝혀졌다. 산화적 스트레스는 p53 전사 반응의 활성화, 시클린-의존성 키나제(cyclin-dependent kinases) 억제제의 조절 및 텔로미어 단축을 통해 노화를 유도하여 궁극적으로 DNA 손상 반응을 유발한다[13]. 이 연구에서, 미토콘드리아 산화 스트레스는 100 μM CsA 처리 후 상승한 것으로 보였다.

세포 내 칼슘 수치는 100 μM CsA 처리에서 감소하였지만, 미토콘드리아 칼슘 수치는 100 μM CsA 처리 후 상승하였다. CsA는 미토콘드리아로부터의 콜라겐 비드-유도된 칼슘 방출을 지연시키는 것으로 알려져 있다[6]. CsA는 세포 내 칼슘 수치를 나타내는 Fluo-3 형광의 2배 증가를 유도한다[40]. 미토콘드리아는 칼슘이온의 항상성 조절에 참여하고 세포질 칼슘 신호를 조절할 수 있다[41,42]. 대사 과정의 칼슘 의존적 조절은 미토콘드리아 매트릭스(matrix)에서 일어난다[43].

GSK3는 세포의 세포질, 미토콘드리아 및 핵에서 발견된다[44]. GSK3은 글리코겐 대사에서 중요한 역할을 하는 글리코겐 합성제(glycogen synthase)를 인산화 및 조절한다[45]. 핵과 미토콘드리아의 GSK3는 세포질에서 보다 더 활동적이다[44]. GSK3β는 신장 허혈 또는 저산소증으로 인한 산화 스트레스로 인한 미토콘드리아 기능 장애와 관련이 있는 것으로 보고되었다[46]. ERK1/2 경로는 세포가 세포 외 신호를 세포 내 반응으로 전환시키는 신호 전달 모듈이다[47]. ERK1/2 경로는 유전자 발현, 혈관 형성, 세포 분화, 증식 및 기능의 조절을 포함한다[47]. ERK의 활성화는 노화 반응을 유발하기 위해 MKK3/6-p38 경로의 자극으로 이어진다[48]. ERK 매개 노화는 세포주기 진행, 미토콘드리아 기능 및 세포 신호 전달에 필요한 단백질의 프로테아좀 의존성 분해를 포함한다[49]. ERK1, ERK2 또는 MEK 억제제는 노화 메커니즘의 활성화를 방지하는 것으로 알려져 있다[49].

이 연구에서 CsA는 GSK3β 신호 경로 및 ERK1/2 신호 경로를 활성화시켰는데, 이는 세포의 노화 반응과 밀접한 관련이 있다. GSK3β의 기능 변화는 노화에 수반되는 현상으로, GSK3β는 에너지 소비와 생산의 균형 조절의 상실을 의미한다[46]. ERK1/2 경로의 활성화는 활성 산소의 증가와 함께 노화를 촉진한다[48]. CsA는 일반적으로 칼슘/칼모둘린(calcium/calmodulin) 의존성 세린/트레오닌(serine/threonine) 단백질 포스파타제인 칼시뉴린(calcineurin)을 억제한다[50]. 칼시뉴린은 세린-9에서 GSK3β를 탈인산화한다[50].

본 연구에서 CsA는 ZO-1 발현을 감소시켰는데 이는 부착성 및 tight junction 접합 단백질이다[51]. CsA는 ZO-1의 하향 조절을 유도하는 것으로 보고되었다[46]. ZO-1 발현은 ERK/p38 MAPK/JNK 경로의 활성화에 의해 조절된다[52]. 결론적으로, CsA는 미토콘드리아-, GSK3β- 및 ERK1/2- 의존적 경로를 통해 사람 각막내피세포에서 노화를 유도하고 ZO-1 발현을 감소시킨다.

본 연구는 배양된 사람 각막내피세포에 대한 비생체 실험으로 생체 내에 적용하기에는 어느 정도 한계가 있을 것으로 생각된다. 또한 각막에 점안제로 사용되었을 경우 각막상피, 각막기질을 통과하여 각막내피에 도달하는 농도는 매우 적을 것으로 기대된다. 그러나 고농도로 CsA가 사용되는 경우에는 각막내피에도 고농도로 도달할 가능성이 있어 이에 대한 부작용이 우려된다. 따라서 생체 내 사람 각막내피세포의 노화에도 영향을 미치는지에 대한 분석을 위해 추후 안정성이 보장된 생체 내 연구가 필요하다.

현재 포도막염 등의 안구염증 질환 및 안구건조증 환자의 치료용 점안액으로 CsA가 사용되고 있다[53]. 본 연구 결과에 따르면 CsA의 농도에 따라 사람 각막내피세포의 노화가 유도되어 고농도 CsA에 의한 치료 효과에는 어느 정도 제한이 있을 것으로 생각된다. 따라서 이와 같은 부작용을 줄이기 위해 고농도의 CsA가 포함된 점안액 사용 전, 환자의 각막내피세포 평가가 이루어져야 할 것이다. 또한 CsA의 치료에서 사람 각막내피세포의 노화는 최소화시키고, 치료 효과를 최대로 얻을 수 있는 방법에 대한 시도와 다양한 연구가 진행되어야 한다.
