다발골수종 치료제인 bortezomib과 연관된 마이봄샘기능부전 1예에 대하여 보고하고자 한다.
59세 여자 환자가 두 달 전부터 심해진 안구건조증과 세안을 해도 닦이지 않는 눈꺼풀 분비물을 주소로 내원하였다. 환자는 5개월 전 다발골수종을 진단받고 bortezomib 치료를 네 차례 받은 상태였다. 세극등현미경검사에서 양안 눈꺼풀에 다수의 마이봄샘 개구부 막힘이 관찰되었고, 마이봄샘촬영 영상에서 위눈꺼풀은 1/3 이상 2/3 미만, 아래눈꺼풀은 2/3 이상의 마이봄샘이 폐쇄되어 있었다. 온찜질을 포함한 눈꺼풀 위생, 점안항생제와 스테로이드, 인공눈물 사용 후에도 8개월간 증상과 병변은 큰 차이를 보이지 않았다. Bortezomib 치료 종료 2개월 후 환자가 안과에 재내원하였을 때, 주관적 증상은 호전되었고, 마이봄샘 개구부 막힘은 모두 사라졌다. 양안의 마이봄샘폐쇄 정도는 큰 차이를 보이지 않았다.
임상의는 bortezomib 치료를 받는 환자에서 마이봄샘기능부전이 발생하거나 악화될 수 있음에 대해 인지하여야 하고, 마이봄샘 기능부전의 치료 방침을 수립하거나 환자를 교육할 때 이를 고려해야 할 것으로 생각한다.
This study reports a case of meibomian gland dysfunction associated with bortezomib, which is a treatment of choice for multiple myeloma.
A 59‐year‐old female patient presented to our hospital with a complaint of dryness that had worsened for 2 months and eye discharges that were difficult to remove even after washing her face. The patient had been diagnosed with multiple myeloma 5 months prior and had undergone four cycles of bortezomib therapy. Slit‐lamp microscopy revealed a number of pouting of the meibomian gland (MG) orifices in both eyes. Meibography revealed that more than one‐third and less than two‐thirds of the total MG area of both upper lids were lost and more than two‐thirds of the total MG area of both lower lids were lost. No clinically significant improvements were noted at 8 months despite thorough eyelid hygiene therapy, including warm compresses, topical antibiotics, steroids, and artificial tears. However, when the patient revisited our clinic 2 months after completing bortezomib treatment, the subjective symptoms had improved and all of the pouting of MG orifices had disappeared. There was no significant difference in the MG dropout area for either eye compared with the observations from a previous visit during bortezomib treatment.
Clinicians should be aware that MG dysfunction may occur or worsen in patients receiving bortezomib treatment and should consider this when establishing a treatment plan for meibomian dysfunction or when educating patients.
마이봄샘에서 분비된 마이봄은 눈물층 내 지방층을 형성하여 눈물증발을 지연시켜 눈물층의 안정성을 증대시키고, 눈 깜박임 시에는 안구 표면을 윤활하는 역할을 한다. 마이봄샘기능부전은 눈물막의 안정성을 낮추고 눈물의 증발을 증가시켜 눈물증발 증가와 관련된 건성안의 주요 원인으로 알려져 있으며, 크게는 마이봄샘 분비물(meibum)의 저분비 혹은 과분비로 분류할 수 있다. 이 중 마이봄샘 분비물 저분비 상태는 분비 자체가 저하되거나 마이봄샘 말단 도관(terminal duct)이 폐쇄되어 발생한다고 알려져 있다[
Bortezomib (Bortevel®, Logenex Pharm GmbH, Hamburg, Germany)은 프로테아좀 억제제(proteasome Inhibitor)로 세포의 항상성을 파괴하여 세포사멸을 일으키는 항암제이다. Bortezomib은 다발골수종(multiple myeloma) 및 외투세포림포종(mantle cell lymphoma)의 치료제로 미국 Food and Drug Administration (FDA)의 승인을 받았다. Bortezomib 의 주요 이상 반응으로는 위장관계 장애, 피로, 말초신경병증, 혈소판감소증, 발진 등이 보고되었으며[
59세 여자 환자가 5‐6년 전부터 존재하던 양안의 안구건조감이 심해지고, 눈꺼풀을 닦아도 분비물이 계속 존재한다고 하여 본원에 내원하였다. 6개월 전 타원 진료기록에는 쉬르머검사 양안 10 mm였고, Lipiview® 눈물간섭계(Ocular Surface Interferometer, TearScience Inc., Morrisville, NC, USA)의 적외선 카메라로 마이봄샘을 촬영하였을 때 양안의 위눈꺼풀은 1/3에서 2/3 사이, 아래눈꺼풀은 2/3 이상으로 폐쇄되어 있었으며(
환자는 본원에 내원하기 5개월 전 다발골수종을 진단받고 bortezomib, thalidomide, dexamethasone 병합 약물 치료를 1개월 간격으로 4회 받은 상태였으며, 이외 다른 전신질환을 진단받은 적은 없었다. 본원 내원 당시 시행한 세극등 현미경검사에서 양안 아래눈꺼풀에 다수의 마이봄샘 개구부 막힘이 관찰되었고(
환자는 11개월 동안 9차에 걸쳐 bortezomib, thalidomide, dexamethasone의 병합 약물 치료를 받았다. Bortezomib 치료가 종료된 지 2개월이 지나고 환자가 안과에 재내원하였을 때, 양안 건조감이 많이 호전되었다고 하였다. 세극등현 미경검사상 양안에서 보이던 다수의 마이봄샘 개구부 막힘은 사라졌으나, 마이봄샘 분비물이 치약과 같이 분비되는 양상은 비슷하였고, 양안 각결막에서 Oxford grading system[
Bortezomib 투약 시 안구에 나타나는 이상증상은 드물게 발생한다고 알려져 있다. Ghobrial et al. [
Bortezomib이 콩다래끼를 발생시키는 기전은 명확하게 밝혀지지 않았으나, Fraunfelder and Yang[
자연적으로 발생한 콩다래끼와 달리 bortezomib 연관 콩다래끼는 보존적 치료에 잘 반응하지 않고 bortezomib 치료를 중단할 때까지 지속되거나 재발된다고 알려져 있다[
본 증례는 bortezomib과 관련된 마이봄샘기능부전에 대한 국내에서의 첫 번째 증례이다. 기존에 보고된 증례들과 본 증례를 종합하여 보았을 때, bortezomib 복용 후 발생하거나 악화된 마이봄샘기능부전은 약을 사용하는 도중에는 고식적인 방법으로 쉽게 치료되지 않고, bortezomib 복용을 중단하였을 때 증상이 호전되는 것을 확인할 수 있었다. 본 연구 결과는 bortezomib을 복용하는 환자들에게 마이봄샘 기능부전 및 콩다래끼 등의 합병증의 경과 및 치료에 관하여 교육을 할 시에 도움을 줄 수 있을 것으로 생각한다.
