2형 당뇨병 환자에서 황반부 미세혈관과 신경섬유층 및 신경절세포-속얼기층 두께의 변화

목적

당뇨망막병증이 발견되지 않은 2형 당뇨병 환자에서 파장가변광원 optical coherence tomography (OCT, swept source OCT [SS-OCT])와 optical coherence tomography angiography (OCTA, swept source OCTA [SS-OCTA])를 이용하여 황반의 신경섬유층(nerve fiber layer, NFL) 및 신경절세포-속얼기층(ganglion cell-inner plexiform layer, GCIPL)의 두께와 황반부의 표층망막혈관밀도 및 심층망막혈관밀도, 맥락막모세혈관밀도 변화를 확인하고자 한다.

대상과 방법

2형 당뇨병으로 진단받은 환자 중 당뇨망막병증이 발견되지 않은 64안을 대상으로 하였으며, 당뇨병이 없는 54안을 대조군으로 하여 비교하였다. 모든 환자에서 SS-OCT와 SS-OCTA를 이용하여 검사를 시행하였다.

결과

황반의 표층 및 심층 망막혈관의 혈관밀도는 환자군과 대조군 사이에서 차이를 보이지 않았다. 하지만 NFL-GCIPL 두께는 환자군에서 대조군에 비해 통계적으로 의미 있는 감소를 나타냈으며 당뇨병의 이환 기간이 길수록 두께가 얇아져 있음을 확인하였다.

결론

본 연구의 결과에서 2형 당뇨병 환자에서 당뇨망막병증 발생의 초기 변화에서 신경변성이 일어남을 추측할 수 있다.

ABSTRACT

Purpose

To investigate thickness of the nerve fiber layer-ganglion cell inner plexiform layer (NFL-GCIPL) complex and vessel density of the superficial and deep retinal vessels and choriocapillaris using swept source optical coherence tomography (SS-OCT) and angiography (OCTA) in type 2 diabetic patients without diabetic retinopathy (DR).

Methods

Sixty-four eyes of type 2 diabetic patients without DR were included. A control group of 54 eyes without diabetes was also recruited. All patients underwent multimodal imaging evaluation using SS-OCT and OCTA.

Results

Vessel density of the superficial and deep retinal vessels was not different between the study and control groups. A significant decrease in NFL-GCIPL complex thickness was observed in the study group compared to the control group. NFL-GCIPL thickness of the macula decreased with increased duration of diabetes.

Conclusions

These results suggest that neurodegeneration might be an early change in the development of DR.

당뇨망막병증은 많은 나라에서 생산 인구의 후천적 시력 손상의 주요한 원인 중 하나로 알려져 있다[1]. 최근 국민건강영양조사 보고에서 당뇨병 환자에서 당뇨망막병증의 유병률은 20%였으며, 생활 습관의 변화로 인해 당뇨병과 그로 인한 당뇨망막병증의 유병률이 증가될 것으로 예상된다[2,3]. 당뇨망막병증은 초기에 시력 변화가 적어 진단이 늦어지고, 진행되면 영구적 시력장애가 발생할 수 있어 조기 진단이 중요하다[4].
당뇨망막병증의 발병기전은 다양하게 제시되고 있다. 고혈당에 의한 망막혈관투과성 증가와 망막조직허혈에 따른 신생혈관형성, 세포내 산화스트레스의 증가로 인한 망막염증반응의 증가, 면역세포 및 신경아교세포의 활성화에 의한 망막신경변성이 당뇨망막병증의 발생 및 진행에 영향을 미치는 것으로 보고되고 있다[5,6]. 하지만 당뇨망막병증 초기의 변화로 미세혈관의 변화와 망막신경변성의 선후관계는 밝혀지지 않았으며 이에 대한 많은 연구가 진행 중이다[4,7].
빛간섭단층촬영술(optical coherence tomography, OCT)과 빛간섭단층혈관조영술(optical coherence tomographic angiography, OCTA)은 비침습적 진단 장비로 망막의 단면과 망막내 미세혈관의 생체내 고해상도 촬영이 가능하게 하였다[8]. 특히 당뇨망막병증 환자에서 OCT와 OCTA의 복합적인 사용은 이 질환의 진단 및 치료 방법의 결정에 있어서 중요성이 증가되고 있으며, 장비의 발달로 인한 환자 맞춤형 치료를 가능하게 하고 있다[9].
본 연구에서는 당뇨망막병증이 안저검사에서 발견되지 않은 2형 당뇨병 환자에서 파장가변광원 OCT (swept source OCT, SS-OCT)와 OCTA (swept source OCTA, SS-OCTA)를 이용하여 황반부의 미세혈관의 변화와 신경섬유층(nerve fiber layer, NFL) 및 신경절세포-속얼기층(ganglion cell-inner plexiform layer, GCIPL)의 변화를 건강한 대조군과 비교하여 초기 당뇨망막병증의 변화를 확인하고자 한다.

대상과 방법

2020년 1월부터 2020년 10월까지 경상대학교병원 안과에서 당뇨망막병증 선별검사를 위해 의뢰되어 당뇨망막병증이 발견되지 않은 2형 당뇨병 환자군 64안과 당뇨를 진단받지 않은 대조군 54안을 대상으로 의무기록을 분석하여 후향적 연구를 시행하였다. 약물로 조절되는 고혈압을 제외한 2형 당뇨병 이외의 전신적인 질환을 가진 환자는 연구에서 제외하였으며, ±3디옵터 이상의 굴절이상을 가진 경우나 1.5디옵터 이상의 난시를 가진 경우, 녹내장, 망막 및 시신경질환을 가지고 있거나 단순 백내장수술을 제외한 안내수술을 받은 환자는 제외하였다. 또한 촬영한 SS-OCT와 SS-OCTA에서 주시 이탈, 망막층 분할 오류가 있는 환자는 연구에서 제외하였다. 본 연구는 후향적으로 의무기록을 분석한 피험자 동의 면제 연구로, 경상대학교병원 생명의학연구윤리심의위원회의 승인을 받았으며(IRB 승인 번호: GNUH 2021-01-003), 모든 연구 과정에서 헬싱키선언을 준수하였다.
모든 환자에서 자세한 병력청취와 최대교정시력, 비접촉성 안압검사, 각막굴절력검사, 세극등현미경검사, 안저촬영 및 Topcon’s DRI Triton® (Topcon Corporation, Tokyo, Japan)를 이용하여 SS-OCT, SS-OCTA를 시행하였다. SS-OCT로 유두주위신경섬유층(peripapillary retinal nerve fiber layer, pRNFL) 두께, ETDRS 원의 황반부 1 mm의 중심황반두께(central subfield macular thickness, CST), NFL-GCIPL 두께(상측, 하측, 이측, 비측)를 자체 내장되어 있는 알고리즘의 소프트웨어(IMAGEnet® 6; Topcon Corporation, Tokyo, Japan)를 이용하여 자동 측정하였으며(Fig. 1A), 중심오목 아래맥락두께(subfoveal choroidal thickness, SFCT)는 소프트웨어의 측정 도구를 이용하여 수동으로 측정하였다. SS-OCTA는 황반중심오목을 기준으로 4.5 × 4.5 mm의 범위를 측정하였으며, 측정값으로는 표층망막혈관밀도 및 심층망막혈관밀도, 맥락막모세혈관밀도를 상측, 하측, 이측, 비측으로 나누어 내장된 소프트웨어(IMAGEnet® 6, Topcon Corporation)에 의해 계산된 값을 사용하였으며(Fig. 1B), 표층모세혈관은 내경계막의 2.6 μm 아래에서 내망상층과 내핵층 경계부의 15.6 μm 아래까지, 심부모세혈관은 내망상층과 내핵층 경계부의 15.6 μm 아래에서 70.2 μm 아래까지, 맥락막모세혈관은 기저막 아래에서 기저막 10.4 μm 아래까지로 정의되었다.
모든 통계적인 분석은 SPSS® 21.0 (IBM Corp., Armonk, NY, USA)을 사용하였다. 당뇨군과 대조군의 비교에서 연속형 변수는 t-검정을 사용하였으며, 범주형 변수는 카이제곱검정을 이용하였다. NFL-GCIPL 두께와 변수의 상관관계의 분석에는 Pearson 검정을 통해 상관계수를 측정하였다. 모든 통계에서 p값이 0.05 미만인 경우 통계적 유의성이 있다고 판단하였다.

결 과

대조군과 환자군의 평균 나이는 각각 54.60 ± 20.38세, 56.22 ± 14.07세로 유의한 차이를 보이지 않았으며, 최대교정시력, 안압 및 구면렌즈대응치도 두 군의 차이를 보이지 않았다. 환자군에서 평균 당뇨 이환 기간은 9.23 ± 6.02년이었으며 당화혈색소는 7.52 ± 1.72%였다. SS-OCT로 측정한 CST와 SFCT는 두 군 간의 차이를 보이지 않았으며, pRNFL 두께 역시 상측, 하측, 이측, 비측 모두에서 두 군의 통계학적인 차이가 나타나지 않았다(Table 1).
SS-OCTA를 이용하여 측정한 황반부의 표층망막혈관밀도는 대조군과 환자군에서 모든 영역에서 차이를 보이지 않았으며(Table 2), 황반부의 심층망막혈관밀도 역시 상측, 하측, 이측, 비측 모두 두 군의 차이를 보이지 않았다(Table 3). 또한 맥락막모세혈관밀도도 황반부의 모든 영역에서 두 군의 통계학적인 유의한 차이가 나타나지 않았다(Table 4).
SS-OCT로 측정한 NFL-GCIPL 두께는 대조군과 환자군에서 황반부 상측 영역은 각각 86.70 ± 9.39, 83.05 ± 7.74 μm, 하측 영역은 86.37 ± 9.40, 83.05 ± 7.33 μm, 이측 영역은 83.20 ± 8.84, 81.25 ± 6.56 μm, 비측 영역은 86.24 ± 8.79, 82.56 ± 7.80 μm로 환자군에서 모두 대조군에 비해 통계적으로 유의하게 감소되었다(p=0.022, p=0.033, p=0.046, p=0.018) (Table 5, Fig. 2).
환자군에서 NFL-GCIPL 두께와 나이, 당화혈색소, 당뇨 이환 기간의 상관관계를 분석하였을 때 나이, 당화혈색소는 황반부 모든 영역의 NFL-GCIPL 두께와 의미 있는 상관관계를 보이지 않았으나, 당뇨 이환 기간과 NFL-GCIPL 두께는 황반부 상, 하, 이, 비측 모든 영역에서 통계적으로 유의한 반비례 관계를 보였다(Table 6, Fig. 3). 또한 고혈압의 여부는 모든 영역의 NFL-GCIPL 두께와 유의미한 관계를 보이지 않았다(p>0.05).

고 찰

본 연구는 안저검사상 당뇨망막병증이 관찰되지 않은 2형 당뇨병 환자와 당뇨병이 없는 환자의 SS-OCT와 SS-OCTA를 이용한 황반부의 NFL-GCIPL 두께와 혈관밀도를 비교한 후향적 단면 연구로 당뇨병 환자의 당뇨망막병증 발생 전의 황반부 변화를 보고자 하였다. 당뇨망막병증의 발생 기전에서 혈액-망막장벽의 파괴, 혈관주위세포와 내피세포의 소실, 신경아교세포의 활성화 등에 의한 망막 미세혈관 장애와 염증 및 면역세포, 신경아교세포의 활성화 등에 따른 신경변성이 알려져 있으며, 이들의 선후관계에 대해 많은 연구들이 진행 중이다[10-14]. 또한 망막의 미세혈관 장애와 신경변성은 신경세포, 신경아교세포, 혈관세포로 이루어진 하나의 단위인 신경혈관단위(neurovascular unit, NVU)에서 상호작용하여 당뇨망막병증을 일으키는 것으로 보는 의견도 있다[6]. 특히 알츠하이머 치매나 파킨슨병과 같은 대표적인 신경변성 질환에서 잘 알려진 대로 신경변성과 신경전달물질에 의해 NVU가 파괴되고 이로 인해 혈관-뇌 장벽이 파괴되는 과정이 당뇨망막병증에서도 유사할 경우 미세혈관의 파괴뿐만 아니라 신경변성도 억제할 수 있는 치료가 당뇨망막병증의 발병에 중요한 치료가 될 수 있음을 시사하기도 한다[6,15-17].
이전의 SS-OCTA를 통한 당뇨망막병증 환자의 황반부 혈관밀도를 비교한 연구들에서 당뇨망막병증이 없는 당뇨병 환자의 오목무혈관부위가 정상인에 비해 넓어져 있었으며, 이는 황반부의 표층 및 심층의 혈관분포 감소와 관련이 있음을 보고하였으며, 혈관감소의 정도가 당뇨망막병증의 진행 정도와 반비례함을 보고하였다[18-20]. SS-OCT를 이용한 당뇨병 환자에서 GCIPL의 손상에 대한 연구에서는 황반부의 GCIPL 두께가 정상인에 비해 감소되며, GCIPL 두께의 감소 속도는 당뇨병 환자에서 정상인에 비해 빠르게 감소됨을 보고하였다[21,22]. 또한 GCIPL 두께의 감소는 황반부 표층혈관밀도가 감소함에 따라 비례적으로 감소함을 보고하였다[23].
본 연구에서 당뇨망막병증이 발견되지 않은 2형 당뇨병 환자와 대조군을 SS-OCTA를 통해 황반부의혈관 밀도를 비교하였을 때 황반부의 표층 및 심층망막혈관 밀도는 이전의 연구와는 달리 모든 영역에서 두 군 간의 차이를 보이지 않았으며, 맥락막모세혈관밀도 역시 차이를 보이지 않았다. 하지만 CMT 및 pRNFL 두께는 차이가 없었으나 NFL-GCIPL 두께는 당뇨병 환자군에서 대조군에 비해 의미 있게 감소함을 확인하였으며, NFL-GCIPL 두께는 나이와 당화혈색소와 무관하였고 당뇨 이환 기간과 상관관계가 있음을 알 수 있었다. 본 연구에서 이전 연구들과의 차이는 황반부의 혈관밀도의 차이를 보이지 않은 점인데, 이는 연구의 제한점인 표본 수의 제한, 당뇨 이환 기간 등의 인자의 영향, 측정 OCTA 장비의 차이 등이 영향을 미쳤을 가능성이 있지만, 실제로 당뇨망막병증의 발생에 있어 황반부의 미세혈관 변화보다 신경변성이 선행한다는 가능성을 생각해볼 수 있다. 2016년 Sohn et al [24]은 동물 및 사후 사람에서 당뇨망막병증의 초기의 변화로 NFL의 두께와 GCIPL 두께의 감소가 선행되고 혈관주위세포와 미세혈관의 변화는 조직학적으로 관찰되지 않음을 보고하며 당뇨망막병증의 미세혈관 변화에 신경변성의 선행 가능성을 보고하였다. 특히 NFL-GCIPL의 두께가 당화혈색소에 영향을 받지 않고 당뇨 이환 기간 반비례하는 점은 이전에 당뇨병조절 및 합병증연구(The Diabetes Control and Complications Trial)에서 당화혈색소와 당뇨 이환 기간이 당뇨망막병증의 진행에 모두 관련이 있지만 최근의 당화혈색소보다 고혈당에 노출된 기간, 즉 당뇨 이환 기간이 더 중요한 지표인 점에 합당한 소견으로 생각된다[25].
본 연구의 제한점으로 후향적 단면적 연구로 적은 환자 수와 환자 선택 오류가 발생할 수 있으며, 2형 당뇨병 환자만을 대상으로 하여 1형 당뇨병에서 당뇨망막병증을 확인할 수 없는 점이 있다. 또한 본 연구에 사용한 SS-OCTA 장치에서는 오목무혈관부위의 자동 측정이 불가능하여 비교할 수 없었던 점이 제한점이며, 형광안저촬영을 통한 주변부 망막혈관 상태를 확인하지 못한 점 및 대상 환자에서 당뇨망막병증 및 망막혈관에 영향을 미칠 수 있는 혼란변수를 통제하지 못한 점은 추후 전향적 관찰 연구를 통해 확인이 필요할 것으로 생각된다.
결론적으로 본 연구를 통해 당뇨망막병증이 없는 2형 당뇨병 환자에서 NFL-GCIPL 두께가 정상인에 비해 감소되었고, 이는 당뇨 이환 기간에 반비례하여 얇아져 있음을 알 수 있다. 또한 황반부 NFL-GCIPL 두께 감소는 초기 당뇨망막병증의 변화로 생각되며 SS-OCT를 이용한 NFL-GCIPL두께의 측정은 초기 당뇨망막병증 발견에 유용하게 사용할 수 있을 것으로 생각된다.
